The Magnolia at Croasdaile Village offers support for residents living with cognitive impairments. It is a licensed Special Care Unit. In addition to spacious apartments, memory-supported living offers a private garden with inviting outdoor seating. Heartfelt Connections is an individualized program focusing on each resident’s capabilities, along with a personalized plan concentrating on each resident’s unique abilities while celebrating each small success.
This remarkable program promotes dignity and a sense of accomplishment in day-to-day activities, supports improved self-worth, self-esteem, and overall health and well-being, and reduces fear, anxiety, and frustration.
For every incoming Magnolia resident, we hold a roundtable meeting that includes the resident, the resident’s family, and our Memory Care team. The purpose is to gather detailed information about the new resident’s needs and desires to develop an individualized care plan.
Heartfelt Connections is a nationally recognized, person centered, activity-focused program that is grounded in the knowledge that the abilities that remain for an individual impacted by Alzheimer’s or other dementias are much more important than what is lost. By leveraging this specialized dementia care, we help loved ones capture a lasting legacy about each resident’s family, their friends, career, hobbies, likes and dislikes, and then apply these insights to individualized programming for the resident. Through this program, we focus on each resident’s unique abilities, enriching lives and creating success stories one small victory at a time.
